What are your thought on biologics use (e.g. Belimumab) in SLE managament?

• In patients not responding to HCQ (alone or in combination with GC) or patients unable to reduce GC below doses acceptable for chronic use, the Biologic Should be added to control disease activity, reduce flares, and allow for GC dose reduction.
• Can be added after a failure on One conventional Immunosuppresive drug.
• Can be delayed after a trial of Two conventional Immunosuppresives

What are your thoughts on Belimumab use in LN managament?

• Should be added early in LN managament, as part of the initial therapy.
• Can be added after (in-adequate response/flares) on initial therapy with (MMF/Cyclophosphamide + - steroids) after 3-6 months.
• Should be considered only in more severe forms of LN.
